Personal Touch Home Health Care Services Inc. is seeking a Hospice RN Primary Case Manager to provide coordinated end-of-life nursing care for patients in home or facility settings.

The role includes conducting comprehensive assessments, developing individualized care plans, managing symptoms, and educating patients and caregivers on hospice philosophy and options. Collaboration with the interdisciplinary team and supporting families through transitions are essential.

Formación

• RN licensure and hospice/end-of-life nursing experience (preferred).
• Strong clinical assessment, care planning, and symptom management skills.
• Excellent communication and teaching abilities for patients and caregivers.
• Ability to work effectively within an interdisciplinary team and provide compassionate support.

Responsabilidades

• Provides coordinated end-of-life nursing care for patients in home or facility settings.
• Conducts comprehensive patient assessments and develops individualized plans of care.
• Manages symptoms and supports comfort-focused outcomes.
• Reviews and updates care plans to reflect patient needs.
• Educates patients and caregivers on hospice philosophy and available treatment options.
• Collaborates with the interdisciplinary team to coordinate services and resources.
• Supports families through care transitions and the hospice experience.
