Job summary

CarePartners Health Services, part of HCA Healthcare, seeks a Primary Case Manager RN in Hospice to deliver high-quality nursing care and coordinate services for patients and families facing end-of-life decisions.

You will collaborate with the IDG, adjust care plans based on assessments, and help ensure comfort and dignity for patients while supporting families throughout the hospice journey.

Qualifications

• Diploma in Nursing, ADN, or BSN required.
• Must hold and maintain current RN licensure with NC Board of Nursing; compact license may apply; BCLS.
• One year of acute care nursing experience required.

Responsibilities

• Provide nursing care and clinical case management for hospice patients.
• Coordinate services with the hospice interdisciplinary team; assess and modify plans of care.
• Support patients and families through end-of-life transitions with culturally sensitive care.
