HCA Healthcare, Inc. is seeking a Hospice RN Primary Case Manager to provide coordinated end-of-life nursing care in patients' homes or facilities. You will perform comprehensive assessments, develop and update individualized plans of care, and manage symptoms to support quality of life.

As part of the hospice interdisciplinary team, you will educate patients and caregivers about hospice philosophy and treatment options and guide families through care transitions and the hospice experience.
