Job summary

Henry Ford Health is seeking a Registered Nurse to serve as Hospice Acute Care Coordinator on a contingent basis. The role will involve coordinating hospice services within multiple Henry Ford and Ascension hospital settings, assessing patient needs, and supporting pain and symptom management.

The ideal candidate holds an Associate's Degree or Diploma in Nursing, is licensed in Michigan, and has hospice experience or exposure.

Qualifications

• Requires an Associate's Degree or Diploma in Nursing.
• Requires understanding of the Hospice concept of care and knowledge of pain and symptom management.
• Demonstrates interpersonal skills to present a positive image of hospice to referrals.
• Demonstrates flexibility to adapt policies to patient/family needs.
• Previous hospice experience preferred.

Responsibilities

• Functions as a care coordinator for Hospice within the hospice setting.
• Assesses and identifies patient needs and coordinates with the interdisciplinary team.
• Provides pain management and symptom control and maintains continuity of care.
• Coordinates with hospital Case Managers, providers, and unit staff for hospice consults/referrals.
