Job summary

Henry Ford Health in Michigan seeks a Hospice Care Coordinator (RN) to assess patient needs, coordinate care, and manage pain and symptoms within the hospice setting. The role supports 40 hours per week with every-other-weekend and holiday rotations.

Requires an Associate's Degree or Diploma in Nursing and a current Michigan RN license; hospice knowledge and flexibility to cover shifts at HF Ascension Novi and other HF Ascension Hospitals are essential. Prior hospice experience is preferred.

Qualifications

• Coordinates with interdisciplinary team to provide complete patient/family care and referral.
• Provides pain management and symptom control.
• Maintains continuity of care by ensuring patient needs are met.

Location

Primary location will be HF Ascension Novi but must be able and willing to cover shifts if needed at all HF Ascension Hospitals.

General Summary

Functions as a care coordinator for Hospice within the hospice setting. Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Provides pain management and symptom control. Maintains the continuity of care by ensuring patient needs are met.

Qualifications

EDUCATION/EXPERIENCE REQUIRED:

• Requires an Associate's Degree or Diploma in Nursing.
• Requires understanding and acceptance of the Hospice concept of care, and a working knowledge of current pain and symptom management.
• Requires interpersonal skills to present a positive image of hospice to those whom are referred to the program.
• Possess the ability to be flexible in adapting policies and procedures to meet the needs presented by hospice patients and families.
• Previous hospice experience preferred.

Certifications/Licensures Required

Requires current licensure in the State of Michigan as a Registered Nurse. Hospice certification preferred.
