Job summary

Henry Ford Health in Detroit, MI is seeking a Hospice Care Coordinator (RN) to join our hospice team on days, 40 hours per week.

You will assess patient needs, coordinate with the interdisciplinary team, provide pain and symptom management, and ensure continuity of care for patients and families.

Michigan RN licensure, an associate degree or nursing diploma, and hospice experience are preferred.

Qualifications

• Associate's Degree or Diploma in Nursing or equivalent.
• Understanding of hospice care concepts and pain management.
• Strong interpersonal skills to present a positive image of hospice.
• Flexibility to adapt policies to patient and family needs.
• Previous hospice experience preferred.

Responsibilities

• Functions as a care coordinator for Hospice within the hospice setting.
• Assesses and identifies patient needs in accordance with hospice care concepts.
• Coordinates with interdisciplinary team to provide complete patient/family care and referral.
• Provides pain management and symptom control.
• Maintains continuity of care by ensuring patient needs are met.
