Qualifications

• Associate's Degree or Diploma in Nursing required.

• Understanding of hospice care concepts and current pain/symptom management.
• Ability to adapt policies to meet patient/family needs; flexible to care for patients across locations.

Responsibilities

• Functions as a care coordinator for Hospice within the hospice setting.
• Assesses and identifies patient needs in accordance with the hospice concept of care.
• Coordinates with interdisciplinary team to provide complete patient/family care and referral.
• Provides pain management and symptom control.
• Maintains continuity of care by ensuring patient needs are met.

Education

Associate's Degree or Diploma in Nursing

Registered Nurse Licensure in Michigan

Hospice Certification Preferred

Hospice care knowledge and pain management understanding

Flexibility to adapt policies for hospice patients and families
