Job summary

Henry Ford Health is seeking a Hospice Care Coordinator to support patients at the primary location with coverage at HF Ascension Novi and other HF Ascension Hospitals as needed. The role emphasizes assessment, symptom control, and coordinating with an interdisciplinary team to ensure comprehensive patient/family care.

The nurse must hold an active Michigan RN license, have Hospice knowledge, and be flexible to adapt policies to patient needs. Hospice experience is preferred and encouraged.

Qualifications

• Requires an Associate's Degree or Diploma in Nursing.
• Requires understanding of the Hospice concept and current pain/symptom management.
• Strong interpersonal skills to present hospice positively to referrals.
• Flexible in adapting policies to patient/family needs.
• Previous hospice experience preferred.

Responsibilities

• Functions as a care coordinator for Hospice within the hospice setting.
• Assesses and identifies patient needs per hospice care model.
• Coordinates with the interdisciplinary team to provide comprehensive patient/family care and referrals.
• Provides pain management and symptom control.
• Maintains continuity of care by ensuring patient needs are met.
