Job summary

Remarkable Hospice is seeking a licensed RN to administer hospice care for patients at home or in facilities across Texas. You will coordinate with a multi-disciplinary team and work independently with minimal supervision.

Responsibilities include assessing patients, developing and implementing care plans, documenting status, educating families, and participating in on-call rotations.

Qualifications

• Licensed RN with a current state license.
• CPR certification required.
• Two years of hospice or clinical RN experience preferred.
• Driver’s license and auto insurance; no OIG exclusions.

Responsibilities

• Transfer knowledge from science into nursing process.
• Maintain nursing competencies per home care standards.
• Administer medications and treatments per policy.
• Develop and implement a Plan of Care with physician orders.
• Document patient status accurately and timely.
• Collaborate with the interdisciplinary team on care plans.
• Educate patients and families on healthcare plans and end-of-life care.
• Participate in on-call rotation and visits.
