Job summary

Remarkable Hospice is seeking a compassionate Registered Nurse to provide in-home and facility-based hospice care. The role involves coordinating with patients, families, and the care team to deliver comfort, dignity, and end-of-life support.

You will perform assessments, develop Plans of Care, administer medications, educate families, and participate in on-call rotations, traveling to patients’ homes as needed.

Qualifications

• RN license in state; CPR certification required.
• Minimum two years of hospice or clinical RN experience preferred.

Responsibilities

• Administers medications and treatments per policy.
• Develops and implements a written Plan of Care with physician orders.
• Completes accurate patient assessments and documents status.
• Coordinates care with multi-disciplinary team and family.
• Participates in on-call rotation and travels to patient homes.
• Maintains patient confidentiality and complies with laws.
