The Care Team Hospice in Champaign, IL seeks a compassionate Hospice RN Case Manager to coordinate patient care, develop individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life.

This full-time role offers Mon-Fri 8:00 AM–4:30 PM with on-call opportunities, on-call compensation, and a comprehensive benefits package including health, 401(k), tuition assistance, and mileage reimbursement.

Qualifications

• Active RN License required.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete scheduled visits, admissions, and interventions with timely documentation in HCHB.
• Respond to patient, family, and caregiver needs, including on-call situations.
• Coordinate care with physicians, caregivers, and interdisciplinary team.
• Educate patients and families on symptom management and end-of-life care.

Schedule & Work-Life Balance

• Monday–Friday, 8:00 AM – 4:30 PM
• On-call opportunities available with additional compensation

Key Responsibilities:

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
