Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Registered Nurse (RN) to join the Admissions Team serving Broward County in Fort Lauderdale, FL. This full-time role works Tuesday–Friday 12:00pm–10:30pm to guide patients and families through hospice care with empathy and clarity.

You will evaluate referrals, educate families, coordinate care with physicians and facilities, and advocate for timely, dignified hospice services across the community.

Qualifications

• Current Florida RN license.
• BSN preferred.
• Hospice certification preferred.
• BLS certification required.

Responsibilities

• Evaluate patients referred for hospice services.
• Educate patients and families on hospice and care continuum.
• Coordinate individualized hospice plans of care with physicians and liaisons.
• Serve as liaison between Trustbridge Hospice and hospitals, facilities and other healthcare professionals.
• Promote hospice philosophy through education and relationship-building.
