Job summary

Patriot At Home in the United States is seeking a licensed RN for full-time home health care serving the Delhi, Cheviot and Westwood area. You will coordinate total patient care, evaluate patient needs, and support families in the home setting.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the state as a Registered Nurse.
• 1+ year of experience in community/home health or hospital preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Criminal background check.
• Current CPR (BLS) certification.

Responsibilities

• Have coordination of total patient care.
• Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
• Perform admission, transfer, recertification, resumption of care, and discharge Oasis for the home care patient.
• All other duties as assigned.

Business Hours: Monday - Friday, 8:00 AM to 4:30 PM

Job Conditions:

• Ability to access patients' homes, which may not be wheelchair accessible.

• Sufficient hearing, eyesight, and physical dexterity for patient assessments and care

• Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
