Job summary

Patriot At Home is seeking a dedicated Registered Nurse for full-time homecare assignments in the Delhi, Cheviot and Westwood area. You will coordinate total patient care, evaluate nursing service effectiveness, and manage admissions, transfers, and discharges in patients' homes.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the state as a Registered Nurse.
• One year or more of community/home health/hospital experience preferred.
• Knowledge of Medicare/Medicaid guidelines.

Responsibilities

• Coordinate total patient care.
• Evaluate the effectiveness of nursing services for patients and families.
• Perform admissions, transfers, recertifications, and discharges for home care patients.
• Execute all other duties as assigned.

The responsibilitiesofourNurses include:

• Have coordination of total patient care

• Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis

• Preform admission, transfer, recertification, resumption of care, and discharge Oasis for the home care patient

• All other duties as assigned

Qualifications:

• Graduate from an accredited School of Nursing

• Licensed in the state as a Registered Nurse

• One or more years of experience in community/home health agency or hospital preferred

• Knowledge of Medicare and Medicaid guidelines

• Criminal background check

• Must have a current CPR (BLS) certification

Job Conditions:

• Valid driver's licenserequired

• Ability to access patients' homes, which may not be wheelchair accessible.

• Sufficient hearing, eyesight, and physical dexterity for patient assessments and care

• Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
