Job summary

Patriot At Home in Mason, OH is seeking a Homecare – LPN (Full Time) to support the RN in delivering patient-centered home health care throughout Mason, West Chester, Fairfield, and Loveland.

You will follow the plan of care, perform procedures under physician orders, document care, and help patients with self-care, with mileage reimbursement and benefits from day one.

Qualifications

• Graduation from an accredited School of Practical Nursing.
• Must be licensed in the State of Ohio as an LPN.
• One or more years in home health experience is preferred.
• Must have a criminal background check.
• Must have a current CPR (BLS) certification.

Responsibilities

• Assist the RN in carrying out the plan of care.
• Assist the RN in performing specialized procedures as directed by the RN under physician's orders.
• Assist with the preparation, implementation, and continuing evaluation of the patient care plan.
• Help teach the patient appropriate self-care techniques.
• Prepare clinical progress notes.
• Assist with the rehabilitation of patients according to the medical care plan.
• All other duties as assigned

Job Conditions:
