Job summary

Patriot At Home in Ohio is seeking a registered nurse for full-time homecare duties in Trumbull County. The RN will coordinate total patient care, assess needs, and collaborate with families to ensure quality home health visits.

The role offers a competitive salary with opportunities to earn extra by taking per diem visits, plus benefits including insurance, a 401K with company matching, and PTO. Valid driver's license required.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the state as a Registered Nurse.
• One or more years of experience in community/home health agency or hospital preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Criminal background check and CPR certification required.

Responsibilities

• Have coordination of total patient care.
• Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
• Perform admission, transfer, recertification, resumption of care, and discharge Oasis for the home health patient.
• All other duties as assigned.
