Job summary

Patriot At Home is seeking a dedicated Registered Nurse to join our home care team in Streetsboro, OH. This full-time role covers a 30 mile radius around Streetsboro, Aurora, Garrettsville and Windham, with weekday hours and patient-centered care.

You will coordinate total patient care, assess nursing services, and support admissions, transfers, and discharges in the home setting while maintaining Medicare guidelines and high-quality documentation.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed in the state as a Registered Nurse.
• One or more years of experience in community/home health agency or hospital preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Criminal background check.
• Must have a current CPR (BLS) certification.

Responsibilities

• Have coordination of total patient care
• Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis
• Preform admission, transfer, recertification, resumption of care, and discharge Oasis for the home care patient
• All other duties as assigned
