Job summary

Patriot At Home in Ohio is hiring a registered nurse for full-time home health care serving Delhi, Cheviot and Westwood territories. You will coordinate total patient care, assess outcomes, and manage admissions, transfers, recertifications, and discharges in the home setting.

We offer a flexible schedule, PTO, mileage reimbursement, and a company 401K with matching. We seek a compassionate RN with valid state license and CPR/BLS certification.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed as a Registered Nurse in the state.
• Current CPR (BLS) certification.

Responsibilities

• Coordinate total patient care.
• Evaluate the effectiveness of nursing services for patients and families.
• Perform admission, transfer, recertification, resumption of care, and discharge Oasis for home care patients.
• Carry out any other duties as assigned.

The responsibilitiesofourNurses include:

• Have coordination of total patient care

• Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis

• Preform admission, transfer, recertification, resumption of care, and discharge Oasis for the home care patient

• All other duties as assigned

Qualifications:

• Graduate from an accredited School of Nursing

• Licensed in the state as a Registered Nurse

• One or more years of experience in community/home health agency or hospital preferred

• Knowledge of Medicare and Medicaid guidelines

• Criminal background check

• Must have a current CPR (BLS) certification

Job Conditions:

• Valid driver's licenserequired

• Ability to access patients' homes, which may not be wheelchair accessible.

• Sufficient hearing, eyesight, and physical dexterity for patient assessments and care

• Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
