Magenta Care LLC in Modesto, CA is seeking a dedicated Home Infusion RN to deliver exceptional patient care in patients' homes. The role requires travel to clients within a 1-2 hour radius and a strong commitment to privacy and dignity.

You will provide direct nursing care, assess and plan treatment, educate patients on infusion therapy, and collaborate with physicians and the care team. A CA RN license and valid driver's license are required, along with reliable transportation.

The primary location for this position will be in Modesto, CA, with the requirement to travel up to one to two hours outside of Modesto to serve our clients.

Responsibilities

• Provide direct patient care, practicing within the scope of the registered nurse practice act.
• Continuously assess, use nursing diagnosis, implement, plan, and evaluate patient care, actively involving patients and their families in the process.
• Collaborate with physicians and other interdisciplinary team members to develop a comprehensive plan of care that addresses the unique needs of each patient and their family.
• Facilitate patient start of care through the admission process.
• Act as a patient advocate, ensuring their rights, preferences, and safety are upheld.
• Document and effectively communicate any changes in the patient's condition promptly to team members, the Director of Clinical Services, and the physician.
• Provide nursing care to home health patients, adhering to established standards of care and following agency policies and procedures.
• Educate patients and their families on infusion therapy management, potential side effects, and self-care practices

Requirements

• Valid CA Registered Nurse license.
• Minimum of 1 year of recent RN experience (Infusion, ER, ICU).
• Excellent IV insertion skills.
• Current California Driver's license with proof of insurability.
• Reliable transportation
