Job summary

Magenta Care LLC is seeking a dedicated Home Infusion Registered Nurse to deliver high-quality patient care in home health settings. The role requires evaluating patients, coordinating with physicians, and guiding families through infusion therapies while maintaining patient privacy and dignity.

You will travel within a 1–2 hour radius of Fremont, CA, to serve clients, with duties that include teaching infusion management, ensuring safety, and advocating for patients throughout the care process.

Qualifications

• Valid CA Registered Nurse license is required.
• Minimum of 1 year of recent RN experience (Infusion, ER, ICU).
• Excellent IV insertion skills.
• Valid California Driver's license with proof of insurability; reliable transportation.

Responsibilities

• Provide direct patient care in home health settings, per RN scope and agency policies.
• Assess patient condition, plan and evaluate care with patients/families involvement.
• Collaborate with physicians and interdisciplinary team to develop care plans.
• Facilitate admission/start of care and advocate for patient rights and safety.
• Educate patients and families on infusion therapy management and self-care.

We are seeking a dedicated and experienced home infusion RN who is passionate about delivering exceptional patient care. The primary location for this position will be in Fremont, CA, with the requirement to travel up to one to two hours outside of Fremont to serve our clients.

Responsibilities

Provide direct patient care, practicing within the scope of the registered nurse practice act. Continuously assess, use nursing diagnosis, implement, plan, and evaluate patient care, actively involving patients and their families in the process. Collaborate with physicians and other interdisciplinary team members to develop a comprehensive plan of care that addresses the unique needs of each patient and their family. Facilitate patient start of care through the admission process. Act as a patient advocate, ensuring their rights, preferences, and safety are upheld. Document and effectively communicate any changes in the patient's condition promptly to team members, the Director of Clinical Services, and the physician. Provide nursing care to home health patients, adhering to established standards of care and following agency policies and procedures. Educate patients and their families on infusion therapy management, potential side effects, and self-care practices.

Requirements
