Job summary

Magenta Care LLC in Oceanside, CA is seeking a dedicated home infusion RN who delivers compassionate, patient-centered care in clients' homes. You will assess, plan, and implement care while coordinating with physicians and the care team.

This role requires an active California RN license, at least 1 year of infusion/ER/ICU experience, strong IV skills, and a valid CA driver’s license with reliable transportation.

• Excellent IV insertion skills are required.
• Valid California Driver's license with proof of insurability; reliable transportation.

Responsibilities

• Provide direct patient care within the RN practice act.
• Assess, plan, implement, and evaluate patient care with patient/family involvement.
• Collaborate with physicians and interdisciplinary team to develop a care plan.
• Facilitate start of care through admission process.
• Advocate for patient rights, preferences, and safety.
• Document changes in patient condition and communicate them promptly.
• Provide nursing care to home health patients per standards and policies.
• Educate patients and families on infusion therapy and self-care.

The primary location for this position will be in Oceanside, CA, with the requirement to travel up to one to two hours outside of Oceanside to serve our clients.

Responsibilities

• Provide direct patient care, practicing within the scope of the registered nurse practice act.
• Continuously assess, use nursing diagnosis, implement, plan, and evaluate patient care, actively involving patients and their families in the process.
• Collaborate with physicians and other interdisciplinary team members to develop a comprehensive plan of care that addresses the unique needs of each patient and their family.
• Facilitate patient start of care through the admission process.
• Act as a patient advocate, ensuring their rights, preferences, and safety are upheld.
• Document and effectively communicate any changes in the patient's condition promptly to team members, the Director of Clinical Services, and the physician.
• Provide nursing care to home health patients, adhering to established standards of care and following agency policies and procedures.
• Educate patients and their families on infusion therapy management, potential side effects, and self-care practices

Requirements

• Valid CA Registered Nurse license.
