Job summary

Enhabit Home Health & Hospice is seeking a registered nurse to coordinate home health services and ensure care aligns with standards. You will collaborate with physicians, patients, caregivers, and field staff to schedule and maintain continuity of care.

Qualified candidates will have an approved nursing program graduate, an active state license, CPR, and at least two years of nursing experience, including home health. In-office presence is required for this role.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid state clinical nursing license or compact license.
• Minimum two years nursing experience in the last 36 months.
• Minimum one year home health experience.
• Knowledge of federal, state, and local laws governing home care operations.
• CPR certification.
• Valid state driver's license and automobile liability insurance.
• Basic technology skills including mobile devices.
• Preferred experience in management and with Homecare Homebase (HCHB).

Responsibilities

• Coordinate home health services to patients per standards and procedures.
• Review patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate communication among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and ensure continuity of care and coordination.
