Enhabit Home Health & Hospice is seeking a nurse leader to coordinate comprehensive home health services, ensuring care aligns with standards and regulatory requirements. You will review patient information, collaborate with physicians and caregivers, and schedule field clinicians to maintain seamless care delivery.

The role requires active nursing licensure, CPR, and home health experience; bilingual abilities and knowledge of HCHB are preferred.

Qualifications

• Graduate of an approved school of professional nursing.
• Authorized license per state requirements, or compact license.
• Two years of nursing experience within the last 36 months.
• At least one year of home health experience.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification.
• Valid state driver’s license.
• Automobile liability insurance as required by law.
• Basic technology skills including operation of a mobile device.
• Preferred: management experience.
• Preferred: Homecare Homebase (HCHB).
• Preferred: bilingual English/Spanish.

Responsabilités

• Coordinate overall home health services to patients per standards and procedures.
• Review and approve patient information for starts of care, recertifications, resumption of care, or evaluations.
• Facilitate relationships between physicians, referrals, patients, caregivers and staff.
• Schedule patients to field clinicians and communicate with field staff, patients, physicians and other providers to maintain care coordination.
