Job summary

Enhabit Home Health & Hospice is seeking a qualified RN nursing leader to oversee the coordination and delivery of home health services, ensuring standards are met and care continuity is maintained.

The role requires an approved nursing degree, active RN license, and at least two years of nursing experience with one year in home health. CPR and a valid driver's license are required, along with basic tech proficiency.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license in current state or compact license.
• Minimum two years of nursing experience within the last 36 months.
• Minimum one year of experience in home health.

Responsibilities

• Coordinate overall home health services delivery per standards and procedures.
• Review and approve patient information at care start, recertification, resumption of care, or evaluation.
• Facilitate communication between physicians, referral sources, patients, caregivers and field staff to maintain care continuity.
