Job summary

Enhabit Home Health & Hospice is seeking an experienced nurse leader to coordinate comprehensive home health services and ensure care meets professional standards. The role involves reviewing patient information at start of care and recertification, and fostering collaboration among physicians, patients, and caregivers.

The ideal candidate has a nursing degree, current RN license, and 2+ years of recent home health experience, with strong communication and regulatory knowledge.

Qualifications

• Graduate of an approved school of professional nursing.
• Maintain a valid clinical nursing license per state requirements.
• Experience practicing under the license for this position.
• Minimum of two years nursing experience within the past 36 months.
• Minimum of one year in home health.
• Knowledge of federal/state/local home care regulations.
• CPR certification and valid driver license may be required.
• Automobile liability insurance as required by law.
• Basic technology skills including mobile devices.

Responsibilities

• Coordinate overall delivery of home health services per standards and procedures.
• Review and approve start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships among physicians, referral sources, patients, caregivers and staff.
• Ensure scheduling of patients to field clinicians and communication to maintain care coordination.
