Job summary

Venture I Inc. is seeking a dedicated Registered Nurse (RN) to join our Home Health and Hospice team. You will deliver skilled nursing care in patients' homes and hospice settings, ensuring safety, quality outcomes, and compassionate support.

The ideal candidate has an active Texas RN license (or compact state), BSN/ADN, CPR (BLS), and reliable transportation. One year of nursing experience is preferred, with strong EMR skills and interdisciplinary collaboration.

Qualifications

Current and valid Registered Nurse license in Texas (or compact state).
Graduate of an accredited School of Nursing (BSN or ADN).
Current CPR (BLS) certification required.
Valid Driver’s License, auto insurance, and reliable transportation.

Responsibilities

Conduct comprehensive initial and ongoing patient assessments.
Develop individualized care plans with physicians, patients, and families.
Administer medications, IV therapies, wound care and other skilled nursing procedures as ordered.
Monitor and report patient progress to physicians and team.
Educate patients and families on disease management and home care techniques.
Coordinate care with interdisciplinary teams including therapists and social workers.
Maintain accurate EMR documentation and charting.
Participate in on-call rotation as assigned.
Ensure compliance with HIPAA and all regulations.
Provide compassionate end-of-life care in hospice settings.
