Job summary

Venture I Inc. is seeking a compassionate Registered Nurse (RN) to join our Home Health and Hospice team in Lufkin and Diboll, TX.

The RN will deliver skilled nursing care to patients at home or in hospice settings, ensuring safety and comfort with patient-centered, interdisciplinary care. The ideal candidate has strong clinical skills, empathy, and communication abilities, plus ability to work independently and manage time efficiently.

Qualifications

• Active Texas RN license or compact state required.
• Graduate from an accredited nursing program (BSN or ADN).
• Current CPR/BLS certification.
• Valid driver's license with reliable transportation.
• Home health or hospice experience preferred.
• Knowledge of OASIS and EMR charting preferred.

Responsibilities

• Conduct initial and ongoing patient assessments.
• Develop individualized care plans with physicians, patients, and families.
• Administer medications, IV therapies, wound care, and other skilled nursing procedures.
• Monitor patient progress and report changes to physicians and care team.
• Educate patients and families on disease management and home care techniques.
• Coordinate patient care with interdisciplinary teams.
• Maintain accurate EMR documentation.
• Participate in on-call rotation.
• Ensure compliance with HIPAA and agency policies.
• Provide compassionate end-of-life care in hospice settings.
