Job summary

Venture I Inc. is seeking a compassionate Registered Nurse (RN) to join our Home Health and Hospice team. The RN will deliver skilled nursing care in patients' homes or hospice settings, ensuring safety, quality care, and comfort.

Strong communication and teamwork are essential for patient-centered care. The ideal candidate has a Texas RN license (or compact state), a BSN/ADN, CPR/BLS certification, and at least one year of clinical nursing experience, preferably in home health or hospice.

Qualifications

Current and valid RN license in Texas or compact state.
Graduate of an accredited School of Nursing (BSN or ADN).
Current CPR (BLS) certification required.
Valid Driver’s License and auto insurance with reliable transportation.
Minimum of one year of clinical nursing experience, preferably in home health or hospice.

Responsibilities

Conduct initial and ongoing patient assessments in home health/hospice.
Develop individualized care plans with physicians, patients, and families.
Administer medications, IV therapies, wound care, and other skilled nursing procedures.
Monitor and report patient progress to physicians and care team.
Educate patients and families on disease management and home care techniques.
Coordinate care with interdisciplinary teams including therapists and social workers.
Document accurately in the EMR and maintain HIPAA compliance.
Participate in on-call rotations as assigned.
Provide compassionate end-of-life care and support to patients and families.
