Job summary

Venture I Inc is seeking a compassionate Registered Nurse (RN) to join our Home Health & Hospice team. You will deliver skilled nursing care to patients in homes and hospice settings, ensuring safety, quality outcomes, and compassionate support.

Strong communication and teamwork are essential. The ideal candidate holds a Texas RN license (or compact state), a BSN or ADN, and CPR (BLS) certification, with solid clinical assessment and documentation skills.

Qualifications

• Current and valid RN license in Texas (or compact state).
• BSN or ADN degree from an accredited school of nursing.
• CPR (BLS) certification required.

Responsibilities

• Conduct comprehensive initial and ongoing patient assessments.
• Develop individualized care plans with physicians, patients, and families.
• Administer medications, IV therapies, wound care, and skilled procedures.
• Monitor and report patient progress to the care team.
• Educate patients and families on disease management and home care.
• Coordinate care with therapists, social workers, and aides.
• Maintain accurate EMR documentation and participate in on-call duties.
• Ensure compliance with HIPAA and agency policies.
• Provide compassionate end-of-life care and support in hospice.
