Job summary

Granite Peak Home Health seeks an experienced Clinical Manager (RN) to lead patient care in the Montana region. You will supervise clinical staff, coordinate care plans, and ensure compliance with CMS, PDGM, and regulatory standards.

Responsibilities include overseeing referrals, budgeting assistance, weekly IDT meetings, and ongoing staff development. Requires Montana RN license, Bachelor's in Nursing, and strong leadership in home health services.

Qualifications

• Bachelor's degree in nursing is required.
• Montana RN license is required.
• Minimum 2 years nursing experience in home health preferred.
• Proficiency with OASIS-E assessment.

Responsibilities

• Oversee all home health patient care within the designated region.
• Supervise staff and coordinate care with CMS and PDGM guidelines.
• Manage referrals, care plans, and patient documentation.
• Lead weekly IDT and leadership meetings.
• Ensure regulatory compliance and staff performance.
• Assist in budgeting and monitoring census, staffing, and expenses.
• Oversee onboarding, training, and annual evaluations.
• Maintain supplies and participate in quality initiatives.
