Job summary

Granite Peak Home Health is seeking a Clinical Manager (RN) to oversee home health patient care within a defined region. You will supervise clinical staff, coordinate assessments, and ensure individualized care plans while aligning with CMS and PDGM guidelines.

Responsibilities include budgeting support, staff development, regulatory compliance, and ongoing quality initiatives. A Montana RN license and 2+ years in home health are highly valued as you lead a dedicated clinical team.

Qualifications

• Bachelor’s Degree in Nursing (Required)
• Montana RN license required
• 2+ years nursing experience in Home Health preferred
• Proficiency in OASIS-E assessment
• Experience with WellSky and web-based systems

Responsibilities

• Oversee all home health patient care in the designated region
• Supervise staff and ensure compliance, quality, and timely care
• Coordinate assessments, care plans, referrals, and patient needs
• Lead weekly IDT meetings and CMS/PDGM-aligned leadership meetings
• Develop and implement policies and procedures for patient care services
• Assist in budgeting and monitor census, staffing, revenue, and expenses
• Ensure regulatory compliance and oversee staff hiring and evaluation
• Oversee onboarding, training, and performance reviews
• Maintain accurate patient documentation and reporting
• Manage supplies and support QA initiatives
• Support organizational goal development and regulatory knowledge
