Job summary

Granite Peak Home Health in Great Falls, MT is seeking a Clinical Manager (RN) to oversee all home health patient care within the region. You will supervise staff, coordinate care plans, manage referrals, and ensure compliant clinical operations.

The role requires a Montana RN license, a Bachelor’s degree in Nursing, and experience in home health. Strong leadership and familiarity with OASIS-E, PDGM, and CMS guidelines are essential.

Qualifications

• Bachelor’s Degree in Nursing is required.
• Montana RN license currently.
• At least 2 years of home health nursing experience preferred.
• Proficiency with OASIS-E assessment.
• Management experience preferred.
• Knowledge of Montana Home Health (DPHHS), CMS guidelines, PDGM and reimbursement programs.
• Ability to exercise initiative and independent judgment.
• Proficiency with electronic health records (WellSky) and web-based systems.
• Must pass background check and not be listed on the OIG exclusions list.
• Must have a cellular phone for work-related use and valid driver's license with reliable transportation.

Responsibilities

• Oversee all home health patient care within the region.
• Provide clinical supervision of assessment, planning, implementation, and evaluation.
• Lead, manage, and hold staff accountable for compliance and timely care.
• Schedule and conduct weekly IDT and leadership meetings.
• Develop and implement processes, policies, and procedures for patient care services.
• Assist in preparing and administering the agency budget.
• Monitor census trends and adjust staffing, revenue and expenses.
