Job summary

Granite Peak Home Health is seeking an experienced Clinical Manager (RN) to oversee home health patient care across a designated region in Montana. You will supervise clinical staff, coordinate assessments, and ensure care plans meet patient needs while maintaining regulatory compliance.

The role requires a Montana RN license, a Bachelor’s in Nursing, and proven leadership in a home health setting. Competitive salary (DOE) with mileage reimbursement and flexible scheduling are offered.

Qualifications

• Bachelor’s Degree in Nursing required.
• Montana RN license required.
• 2 years nursing experience in home health preferred.
• Proficiency in OASIS-E assessment.
• Management experience preferred.
• Knowledge of Montana Home Health (DPHHS), CMS guidelines, PDGM, and reimbursement programs.
• Proficiency with electronic health records (WellSky) and web-based systems.
• Must pass background check and not be listed on the OIG exclusions list.
• Must have a cellular phone for work-related use.
• Valid driver’s license, reliable transportation, and motor vehicle insurance.

Responsibilities

• Oversee all home health patient care within designated region.
• Direct clinical activities and supervise staff.
• Coordinate assessments, care planning, and care delivery.
• Ensure regulatory compliance and documentation.
• Lead weekly IDT meetings and routine leadership meetings.
• Develop and implement processes, policies, and procedures for patient care services.
• Assist in preparing and administering the agency budget.
• Monitor census trends and adjust staffing, revenue, and expenses.
