Job summary

Granite Peak Home Health is seeking an experienced Clinical Manager (RN) to lead home health services across the designated Montana region. You will supervise clinical staff, coordinate care plans, and ensure compliance with CMS/PDGM guidelines.

You will oversee scheduling, budgeting, and quality initiatives while maintaining patient-centered care and strong operational performance. Remote travel within the service area is required.

Qualifications

• Bachelor’s Degree in Nursing is required
• Montana RN license is required
• Minimum 2 years nursing experience in Home Health preferred
• Experience with OASIS-E assessment
• Knowledge of Montana DPHHS, CMS guidelines, PDGM and reimbursement programs
• Proficiency with electronic health records and web-based systems
• Must pass background check and not be listed on the OIG list
• Must have reliable transportation and valid driver’s license

Responsibilities

• Oversees patient care within the designated region
• Manages referrals and care coordination
• Direct supervision of assessment, planning, implementation and evaluation of patient care
• Leads staff, ensures compliance and quality and timely care
• Schedules weekly IDT and leadership meetings per CMS/PDGM
• Develops and implements policies for patient care services
• Assists in budgeting and monitoring census, staffing, revenue and expenses
• Ensures regulatory compliance and staff HR functions
• Oversees onboarding and ongoing staff education
• Performs quarterly check-ins and annual performance reviews
• Maintains accurate patient documentation and reporting
• Maintains clinical supplies and supports QAPI initiatives
• Helps develop organizational and clinical goals
• Monitors regulatory changes in home health practices
