Granite Peak Home Health is seeking a Clinical Manager (RN) to oversee all home health patient care within the regional footprint. You will supervise clinical staff, coordinate care, manage referrals, and ensure individualized care plans to meet patient needs.

The role requires a Bachelor’s in Nursing, Montana RN license, and knowledge of CMS/PDGM guidelines. Strong leadership and strict regulatory compliance are essential; a path to growth in a reputable home health provider is offered.

Qualifikationen

• Bachelor’s Degree in Nursing is required.
• Current Montana RN License is required.
• Minimum of 2 years nursing experience in Home Health is preferred.
• Proficiency with OASIS-E assessment is required.
• Knowledge of Montana Home Health (DPHHS), CMS guidelines, PDGM, and reimbursement programs.
• Ability to exercise initiative and independent judgment.

Aufgaben

• Oversee all home health patient care within the designated region.
• Supervise staff and manage clinical activities for quality and compliance.
• Direct assessment, planning, implementation, and evaluation of patient and caregiver care.
• Lead hiring, evaluation, and termination of personnel.
• Coordinate weekly IDT meetings and leadership meetings per CMS/PDGM guidelines.
• Develop and implement processes, policies, and procedures for patient care services.
