Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage outpatient care under supervision of an RN or MD.

You will educate, coach, and support treatment decisions, collaborate with a Care Advocate and Social Worker, and promote high-quality, compassionate care in home health and hospice settings.

Qualifications

• Current RN license valid for the assigned state(s).
• 1+ year of clinical experience as an RN.
• Current CPR certification is required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so
• Promotes patient health and independence through teaching and appropriate rehabilitative measures
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Regularly communicate patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities and on-call services to patients/families as assigned
• Participates in clinical development and continuing education programs
• Other duties as assigned
