Job summary

UnitedHealth Group, through its Optum family, is seeking a Nurse Case Manager in Fredericksburg, VA to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge planning, and manage outpatient care under supervision of an RN or MD.

You will collaborate with a Care Advocate and Social Worker, provide education and coaching, and participate in ongoing clinical development.

Qualifications

• Current RN license, state(s) you are assigned to work.
• 1+ year of clinical experience as an RN.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures; teaches self-care.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services as assigned.
• Participates in clinical development and continuing education programs.
