UnitedHealth Group is hiring a Nurse Case Manager in Fredericksburg, VA. You will assess patients, coordinate care across the continuum, support discharge planning, and manage outpatient care under supervision. You’ll educate and coach patients and collaborate with a care advocate and social worker to ensure appropriate services.

The role emphasizes on-call coverage, documentation accuracy, and ongoing professional development within a supportive team.

Formación

• Active RN license for the state(s) you are assigned to.
• 1+ year of clinical RN experience.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsabilidades

• Assess patients and coordinate care with the care team.
• Guide discharge and transition planning.
• Provide education, coaching, and treatment-decision support.
• Coordinate ongoing outpatient care under supervision of RN/MD.
• Communicate patient progress to clinical manager and team.
• Assist with staff education and on-call responsibilities.
• Supervise LPNs/HHAs as assigned.
• Complete documentation timely and accurately.
