Job summary

Sequoia Hospice is seeking an experienced Registered Nurse (RN) to join our hospice care team on weekends across Alameda, Santa Clara, and the Peninsula. You will plan and coordinate care, serve as case manager, and educate patients and families in home settings.

Candidates must hold an active CA RN license, have at least 2 years of nursing experience (hospice or home health preferred), CPR certification, and a valid driver’s license with a reliable vehicle.

Qualifications

• Active CA RN License.
• Minimum 2 years of nursing experience (hospice or home health preferred).
• Current CPR certification.

Responsibilities

• Plan, organize, and direct hospice care.
• Develop and revise individualized hospice care plans.
• Administer medications and treatments as prescribed.
