Job summary

Capital City Nurses, in partnership with Care Advantage, seeks a highly capable Home Care Supervisor (RN) to provide clinical leadership in Abingdon VA, Wytheville VA, and surrounding areas. This role focuses on patient-centered home care, caregiver development, and program quality.

The supervisor will lead multidisciplinary teams, conduct assessments, oversee care plans, and ensure regulatory adherence while supporting growth of the home health program.

Qualifications

• Active RN license in Maryland and/or Washington, DC (Compact License accepted where applicable).
• ADN or higher degree required.

Responsibilities

• Lead and supervise home care teams in the Abingdon VA and Wytheville VA service areas.
• Conduct patient assessments, admissions, and develop plans of care.
• Oversee documentation, care plans, and regulatory compliance.
• Coach caregivers and nursing staff to meet quality standards.
