Care Advantage, Inc. in Wilmington, DE is seeking a dynamic RN Home Care Supervisor to lead clinical operations for patients receiving care at home.

This role requires hands-on leadership, in-home oversight, and collaboration with physicians, caregivers, and families to ensure high-quality outcomes. The ideal candidate will have an active RN license (VA Compact accepted), ADN or higher, at least one year of nursing experience, and strong leadership and communication skills.

Qualifications

• Active RN license (VA Compact accepted where applicable)
• ADN or higher required
• Minimum of 1 year nursing experience
• Strong leadership, communication, and interpersonal skills
• Willingness to travel throughout the service territory
• Proficiency with electronic documentation and technology

Responsibilities

• Conduct comprehensive patient assessments and admissions.
• Develop and oversee personalized plans of care.
• Provide clinical oversight and mentor caregivers.
• Conduct in-home supervisory visits and ensure service quality.
• Coordinate with physicians and multidisciplinary team to achieve goals.
