Capital City Nurses, a Care Advantage Company, seeks an experienced Skilled Services Home Care Supervisor (RN) to lead clinical excellence in our Montgomery County service area. This role moves nursing beyond bedside care, mentoring clinicians and shaping outcomes in home-based care.

The supervisor will oversee admissions, assessments, care planning, and supervision of field RNs/LPNs; ensure HIPAA-compliant documentation and regulatory standards; collaborate with physicians to grow the program

Qualificações

• Active Maryland RN License (DC and/or VA a plus).
• 2+ years of nursing experience.
• Experience supervising, mentoring, or leading clinical staff.
• Strong assessment, care planning, and clinical decision-making skills.
• Valid driver's license and reliable transportation.
• Excellent communication, organizational, and documentation abilities.

Responsabilidades

• Conduct admissions, assessments, recertifications, and change-in-condition visits.
• Develop and oversee individualized care plans.
• Monitor patient progress and intervene when clinical needs change.
• Collaborate with physicians, caregivers, and interdisciplinary teams to achieve outcomes.
