You will serve as the clinical leader, mentor, and advocate while ensuring high-quality, compassionate home-based care for patients and families. The role emphasizes nursing leadership, care plan development, caregiver guidance, and regulatory compliance.

Qualifications

• Active RN license in Virginia (compact/multi-state accepted).
• Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable).
• Associate's Degree in Nursing or higher.
• Strong communication and presentation skills.
• Ability to work collaboratively across teams.
• Reliable transportation for home visits (mileage reimbursed).
• Comfortable with technology (computer/iPad documentation).
• Willingness to travel regularly within assigned area.

Responsibilities

• Meet new patients and onboard; explain services and complete start-of-care assessments.
• Develop tailored care plans addressing medical needs, personal preferences, and family involvement.
• Monitor patient progress and report changes to physicians.
• Teach or supervise family members assisting with care.
