Qualifications

• RN with 3-4 years’ experience, 1-2 in home care preferred.

Responsibilities

• Screen and complete referrals to ensure patient meets admission criteria.
• Verify home health benefits and obtain authorizations.
• Ensure physicians initial home care orders are obtained.
