Qualifications

• BSN is required.
• Illinois Registered Professional Nurse license is required.
• American Heart Association Basic Life Support (BLS) certification required.
• Strong interpersonal skills and ability to collaborate with others.
• Maintains active driver’s license and auto insurance.
• ADN acceptable if committed to obtain a BSN within 3 years of employment.

Responsibilities

• Assess and record patient status at admission or point of care.
• Coordinate interdisciplinary care and organize health data.
• Identify problems requiring immediate attention and plan care.
• Perform joint home visits with Charge Nurse and document plans.
• Initiate discharge planning at admission and update as needed.
• Communicate with patient/family and health team; provide education.
• Document care plans, assessments and patient responses.
• Review patient data regularly to update the Plan of Care.
• Refer for continuity of care when needed.
• Participate in multidisciplinary meetings for feedback.
