Job summary

WellMed in Dallas, TX, is seeking a Registered Nurse Healthcare Coach to support high-risk patients in navigating the healthcare system. You will educate, advocate, and collaborate with multidisciplinary teams to improve quality of life and outcomes.

The role emphasizes patient education, medication reconciliation, and care coordination across transitions from inpatient to outpatient settings. A valid Texas RN license and BLS are required, with 2+ years in clinical settings.

Qualifications

• Bachelor's degree in Nursing or Associate's degree with 4+ years of nursing experience.
• Registered Nurse with an active and unrestricted license in the state of employment
• Current BLS certification
• 2+ years of experience in a physician's office, clinical or hospital setting
• Experience interacting productively with multidisciplinary teams
• Knowledge of chronic diseases (COPD, diabetes, CHF, IHD)
• Proficient computer skills to work with electronic medical records

Responsibilities

• Works with providers and clinic staff to identify high-risk patients
• Supports longitudinal care of patients with chronic conditions including health assessments and medication reconciliation
• Conducts motivational interviewing and self-management goal setting
• Provides patient education and creates referrals
• Documents in the electronic medical record accurately and timely
• Participates in daily huddles and care coordination meetings
