Job summary

WellMed, part of the Optum family, seeks an RN Healthcare Coach in Seguin, TX to support high-risk patients through education, advocacy, and care coordination. You will collaborate with physicians, nurses, pharmacists, and social workers to enhance quality of life and empower patients and families.

This role involves navigating the healthcare system, performing medication reconciliation, and facilitating transitions from inpatient to outpatient settings.

Qualifications

• RN license active in state of employment
• Current BLS certification
• 2+ years in physician's office, clinical or hospital setting
• Experience with multidisciplinary teams
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD)
• Proficient with electronic medical records
• TB screening and immunity documentation required

Responsibilities

• Identify patients at high risk with providers and clinic staff
• Support longitudinal care: assess conditions, reconcile meds, educate, refer to resources
• Assist in transitions from inpatient to outpatient with contingency planning and post-discharge needs
• Coordinate care plans with providers and update plans of care
• Document accurately in the EMR and support PCC meetings
• Participate in daily huddles and weekly care coordination reports
