WellMed in Seguin, TX, seeks an RN Healthcare Coach to support high-risk patients and improve care coordination within a multidisciplinary team. You will educate, advocate, and empower patients to manage chronic conditions and navigate the healthcare system.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience.
• Registered Nurse with an active and unrestricted license to practice in the state of employment.
• Current BLS certification.
• 2+ years of experience in a physician's office, clinical or hospital setting.
• Experience interacting productively with individuals and with multidisciplinary teams.
• Knowledge of chronic diseases, especially COPD / asthma, diabetes, CHF and IHD.
• Proficient computer skills to work efficiently with electronic medical records.
• TB screening and immunity proof requirements as per policy.

Responsabilités

• Works with providers and clinic staff to identify patients at high risk.
• Supports longitudinal care of the patient with chronic conditions with assessment, medication reconciliation, and education.
• Coordinates transition from inpatient to outpatient settings and creates referrals.
• Coordinates with providers to establish or update the care plan.
• Performs accurate and timely documentation in the EMR and participates in PCC meetings.
• Prepares reports for weekly meetings and maintains current nursing practice knowledge.
