Job summary

Avamere Rehabilitation of Issaquah is seeking a Registered Nurse (RN) to provide comprehensive patient care in a skilled nursing setting. You will assess residents, administer medications, monitor health, and coordinate care with the interdisciplinary team, while supervising LPNs and CNAs.

Qualifications

• Must hold nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an RN in this state. BSN preferred.
• Must maintain and have an active CPR/BLS during employment.
• Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
• Experience in electronic medical records documentation.

Responsibilities

• Assess residents and develop individualized care plans with the interdisciplinary team.
• Develop and update care plans based on residents' changing needs.
• Administer medications and monitor responses, documenting any adverse effects.
• Monitor vital signs and report changes to the physician.
• Supervise and train LPNs and CNAs; provide orientation to new staff.
• Communicate with residents, families and healthcare professionals; participate in team meetings.
• Engage in quality improvement initiatives and apply evidence-based nursing practices.
