Job summary

East Bay Post Acute is seeking a compassionate Registered Nurse (RN) to join our full-time AM shift nursing team. You will provide comprehensive nursing care, support clinical operations, and collaborate with an interdisciplinary team to ensure resident health and safety.

Responsibilities

• Communicate with physicians, therapists, residents and families.
• Complete accurate and timely clinical documentation.
• Participate in admissions, transfers, discharges and care plan meetings.
• Promote infection control and compliance with regulations.
• Educate residents and families about treatment plans and medications.

• Provide comprehensive nursing care in accordance with physician orders, individualized care plans, and facility policies.
• Perform nursing assessments and monitor resident conditions, identifying and responding appropriately to changes in health status.
• Administer medications, treatments, and nursing procedures safely and accurately.
• Supervise and support LVNs and CNAs in the delivery of quality resident care.
• Communicate with physicians, therapists, residents, families, and interdisciplinary team members.
